Care Coordinator

Roles & Responsibilities:
The Care Coordinator is responsible for coordinating care and providing guidance to primarily low-risk patients. Develops effective interpersonal relationships with patients and works collaboratively with the interdisciplinary care team to improve health outcomes. Utilizes internal and community resources, electronic medical record and Centers for Medicare and Medicaid data to educate patients and form a care plan with specific health outcomes.
Obtains referrals and/or precerts for patients for internal and external appointments and/or orders
Communicates information pertaining to referrals and/or precerts to patients, external medical offices, and internal staff as per established protocol.
Daily review and processing of Epic referral work queue.
May coordinate and assist patients to overcome any obstacles that may inhibit their care plan such as child care and transportation.
Initiates and performs ongoing review of policies related to services provided. Troubleshoots and updates as necessary.
May schedule patient for follow up appointment as needed. Provide patient with guidelines for requesting medical records, if necessary.
May initiate reminder phone calls for next day appointments.
Refers patients to health system and community resources to ensure patients have appropriate resources to overcome barriers to care (e.g. transportation, home care ,durable medical equipment, pharmacy, housing, legal). Researches additional resources to expand knowledge and make appropriate referrals.
Requirements:
Education:
Bachelor's degree required, preferably in a health related profession
Experience:
None required, some experience in healthcare or related field preferred. Bi-lingual candidates (English/Spanish) are highly desirable.
Ability to learn hospital ambulatory scheduling system, electronic medical record and new applications used for care coordination. Must have excellent oral and written communication skills and ability to foster a supportive environment for patients.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
