Nurse Practitioner-Cerebrovascular Center

Roles & Responsibilities:
A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner provides acute/primary health care in accordance with written policies approved by nursing and medical.
1 Assesses the health status of patients by means of comprehensive health history and physical examinations.
2. Initiates therapeutic and diagnostic procedure, consultations, angiographic procedures, and laboratory examinations. Interprets diagnostic findings and accordingly alters plan of care including but not limited to prescribing and altering medication regimens and blood tests according to agreed-upon collaborative practice agreement and protocols in designated area.
3. Promptly identities changes in patient's condition and modify treatment regimen and therapies appropriately to promote optimal health status and outcomes.
4. Provides teaching and counseling in areas of health promotion and maintenance and in disease prevention in collaboration with Stroke Center Administrative Director, the Cerebrovascular Center MSQ Site Director, and other Stroke Team members.
5 Provides pre-, intra- and post-op counseling on care and recovery process for invasive procedures.
6 Clearly and comprehensively documents findings and plan of care in patient's medical record, including but not limited to daily progress notes, procedural reports, admission H&P's, and discharge summaries.
7. Collaborates with Administrative Director, Cerebrovascular Center, ED, and ICU providers regarding safe transfer of stroke patients to higher level of clinical care as deemed necessary.
8. Gathers and prepares statistical reports for data analysis, as needed, for tracking and reporting for stroke core measure performance improvement initiatives and routine maintenance of the stroke database
9. Consults with physician(s), Cerebrovascular Center, and Stroke Team members as established in the collaborative practice agreement.
10. Maintain knowledge current by self-directed learning and attendance at educational Programs and at least 8 hours of CME credits in stroke care management, prevention and related areas per year.
11. Participates in defining, maintaining, and interpreting standards of nursing practice.
12. Interfaces with Nurse Managers/Directors/ PA's/ Front Line nurses
12. Will assume a holistic approach in anticipating patient's needs when initiating referrals to specialty services and will make recommendations that fosters continuity of care specific to patient's therapeutic goals.
13. Initiates hospital admission and discharge after collaborative consultation with Cerebrovascular Center physicians.
14. Works collaboratively with Administrative Director, Cerebrovascular Center MSQ Site Director, and interdisciplinary team to promote optimal patient outcomes.
Requirements:
Graduate of an accredited Nurse Practitioner Program
National Certification as a Nurse Practitioner - ANCC or AANP
Current NYS licensure as Registered Professional Nurse
Current NYS Licensure as a Nurse Practitioner
Current Certifications in BLS, ACLS, PALS
Current DEA Certification or eligibility to apply
Current ED / Critical Care / Acute Care experience
Current certification as a Nurse Practitioner as follows:
a. Is a graduate NYS registered Nurse Practitioner program OR equivalent OR is certified as a Nurse Practitioner by a national association via examination
b. Has completed a pharmacy component of not less than three semester hours or the equivalent and has been instructed in NYS and federal laws relating to prescriptions and record keeping (3 hour course)
c. Has a collaborative agreement with a physician and designated protocols, both filed with the NYS Department of Education

Previous progressive experience relevant to area of clinical practice

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
