Responsible for delivering nursing care to residents and for supervising the care delivered by Home Health Aides (HHAs) within the scope of the Licensed Practical Nurse (LPN) in the State of New York.
Accountable for excellence in resident care and service delivery.
Provides outstanding customer service to all residents, family members, responsible persons, and visitors to the residenceCoordinates and delivers nursing care within the scope of a Licensed Practical Nurse (LPN) including but not limited to catheter care, wound care, dressing changes, nebulizer treatments, ostomy care, and management of medical equipment; medication management including administration of oral medications, injections or PRN medicationsManages and administers medicationsMaintains positive collaborative relations with residents, families and provider partnersReports any significant incidents and/or changes in the resident's condition to the Senior Director of Integrated Care (Director of Nursing), Primary Care Provider and family as indicatedReviews each assigned resident's Individualized Service PlanObserves and reports any significant symptoms or changes in resident's condition to supervising Registered Nurse (RN)Assist in training and orientation of staffDocument care and services provided in the electronic health record using nursing best practices and documentation standardsAttend and participate in scheduled in-service programs, training programs and staff meetingsAssist with scheduling, staffing, and coaching of Resident Care staff as neededParticipate in health, wellness and lifestyle programs and assist with all aspects of resident care and resident experience as neededMay perform other duties as assignedEducation/Experience/Licensure/Requirements:
Must be a Licensed Practical Nurse, licensed to practice in the state of New York with one of the following:
Associates degree and a minimum of two (2) years of full time, or the equivalent of full time, clinical experience in nursing, at least one (1) year of which shall be in a home health care agency, hospital, skilled nursing facility, or community health program that included care of older adults at homeA diploma or associates degree in nursing and at least four (4) years of full-time, or the equivalent of full-time, clinical experience in nursing within the past ten (10) years, at least one (1) year of which shall be in a home health care agency, hospital, skilled nursing facility, or community health program that included care of the older adults at homeBasic first aid and CPR certificationExperience in assisted living, post-acute or long term care setting preferredExperience and/or special interest working with seniorsMust be able to actively communicate in English in all formatsMust be effective in handling problems in the workplace, including anticipating, preventing, identifying, and solving problems as necessaryMust work well in stressful, high pressure situations and maintain composure and objectivity under pressureMust be able to prioritize departmental functions in order to meet due dates and deadlinesRequired to work weekends and holidays as neededRequired to work extended hours and to report to work during times of need including inclement weather and when building concerns ariseCriminal background check requiredHow is the Inspir experience truly brought to life?
